Your benefits elections premiums will be deducted via convenient payroll deductions.

Benefit Levels

M/I Homes believes benefit cost sharing should correspond with income level and classifies employees into the following Benefit Levels based on annual base salary.

Non-Commissioned Employees

Based on regular base salary as of December 31st. Levels will remain constant throughout the following calendar year.

Commissioned Employees, Including Sales Employees

The greater of base salary (if applicable) and net commissions for the previous calendar year or annualized draw amount.

Medical Insurance Biweekly Contributions

Employees who are hired before October 1st of a year will automatically qualify for the Wellness Premium Discount for their year of hire. Employees who are hired after October 1st of a year will automatically qualify for the Wellness Premium Discount for both the year of hire and the next year. In the following years, you and your covered spouse will be required to complete the annual wellness requirement, such as a biometric screening. If you and/or your covered spouse are unable to complete the wellness requirement, you may qualify for an opportunity to earn the same Wellness Premium Discount by different means. Contact benefits@mihomes.com and we will work with you or your covered spouse (and if you wish, your or your covered spouse’s doctor) to establish a reasonable alternative standard that is right for you or your covered spouse in light of your or your covered spouse’s health status. Establishing and satisfying any reasonable alternative standard will not cost you and/or your spouse any money. Upon completion of either the biometric screening or a reasonable alternative standard by both you and your spouse, you will receive a refund of the increased premium amount you paid during the year, and you will receive the credit for the remainder of the Plan Year.

Eligibility for the Wellness Premium Discount is evaluated annually. An employee and/or covered spouse who does not complete the required wellness activity will be required to satisfy a reasonable alternative standard annually if they want to earn the Wellness Premium Discount.

Tobacco Surcharge

If either you or your covered spouse use tobacco products, you will be charged a flat tobacco surcharge of $78.46 per paycheck unless you complete a reasonable alternative standard.

If you and/or your covered spouse are a tobacco user, you may qualify for an opportunity to avoid the surcharge by different means. Contact benefits@mihomes.com and we will work with you or your covered spouse (and if you wish, your or your covered spouse’s doctor) to establish a reasonable alternative standard that is right for you or your covered spouse in light of your or your covered spouse’s health status (for example, completion of a smoking cessation program). Establishing and satisfying any reasonable alternative standard will not cost you and/or your spouse any money. If you and/or your covered spouse satisfy the reasonable alternative standard, the tobacco surcharge will be waived, and you will pay the same amount for medical coverage as a non-tobacco user. Upon completion of either the non-tobacco affidavit or a reasonable alternative standard by both you and your spouse, you will receive a refund of the increased premium amount you paid during the year and the tobacco surcharge will be waived for the remainder of the Plan year. You and your covered spouse do not need to cease tobacco use to avoid the tobacco surcharge; you and/or your covered spouse only need to satisfy the reasonable alternative standard.

Tobacco use is evaluated annually. An employee and/or covered spouse who uses tobacco will be required to satisfy a reasonable alternative standard annually if they want to avoid the tobacco surcharge.
