Stay In-Network to Avoid Surprise Bills
Our dental plan contracts with providers in the Delta Dental Premier or Delta Dental PPO networks at lower, negotiated rates. This means you pay less when you use a provider in one of these networks. If you go out-of-network, the dental plan will pay up to an allowed amount, and you may be billed for additional costs.

ID Cards
Be on the lookout for a paper ID card in the mail. To download a copy, access your Delta Dental account at www.deltadentaloh.com. You’ll need to register your account if it’s your first time signing in.
If you don’t have your ID card with you, let your provider know you’re covered by Delta Dental of Ohio (with nationwide coverage), and they can look you up by your Social Security Number.
